Barnet Community Housing support is a flexible support service that helps you to manage your affairs and to encourage independent living. Our Housing Support worker is specially trained to provide help and advice to vulnerable people in their own home. The support offered is available for as long as it is needed.

Who we serve?

We assist vulnerable Chinese residents of Barnet Council with their housing related issues. The service provides support and help to those who are homeless or living in temporary/permanent accommodation and are affected by mental health issues, physical/learning disability, domestic violence or issues related to the elderly.

We Provide:

Support that promotes independent living, including:
•    Assistance with learning independent living skills
•    Advice and assistance with claiming welfare and housing benefits
•    Advice and assistance with budgeting and managing your bills
•    Assistance with reporting home repairs and managing your tenancy issues
•    General support with day to day living
•    Assistance with arranging personal care and contacting other agencies involved in your care and welfare
•    Physical health issues
•    Supporting and encouraging service users in making and maintaining contact with other services

Your support worker will discuss this with you and will draw up a plan that sets out what support will be provided and how and when that support is delivered. The level of support can increase or decrease as your needs change.

How can I access the service?

Community Housing Support in Barnet can be accessed via a referral form from:
•    Your GP
•    Care manager, social worker or psychiatrist
•    The supporting team at your local authority
•    Any advice or supporting people team at your local authority
•    Any advice or support agency
•    Or, you can apply directly

「華心會」的「社區住屋支持方案」,乃是幫助這些需要幫助的人能夠獨立生活的一項服務。

我們負責的專員皆受過特別的訓練,會盡可能地提供幫助與建議給這些無力生活的人,讓他們找到一個可安心居住的家,並能夠獨立生活。

這項服務的對象是誰?

這項服務的對象是為在巴納區無家可歸的華人,與雖有暫時或長期的住處但飽受精神問題困擾的華人。此外,我們也為肢體與學習障礙或有其他問題的老人提供這項服務。「華心會」提供幫助,讓這些人可以獨立生活的主要服務,包括:

  • 協助學習獨立生活的技能。
  • 為申請社會福利和住屋津貼提供協助及建議
  • 為管理開支協助及建議
  • 協助報告房屋維修或租務
  • 日常生活支援
  • 聯絡其他相關機構,協助安排個人護理服務
  • 身體健康問題
  • 支援及鼓勵服務使用者與其他相關服務保持聯絡

負責提供支援服務的同事將會與你一起討論並制定一個支援計畫, 包括提供如何提供服務,服務的類型和時間等。服務會隨著你地需要而做出相應的調整。

「社區住屋支持」,可以透過以下轉介方式取得:

  • 家庭醫生
  • 支援經理,社工或精神科醫生
  • 社區支援小組
  • 任何在社區提供建議及支援的人士
  • 任何諮詢或支援機構
  • 或者透過個人直接申請
